July Green: Instituto Mário Penna embraces head and neck cancer prevention campaign

Julho Verde arrived and with it a very important action embraced by Instituto Mário Penna: the campaign to prevent head and neck cancer. This is the name given to tumors that involve the airway-digestive tract region, such as tonsils, mouth, cheek, pharynx, gums, larynx, tongue and paranasal sinuses, and skin tumors in the face and neck region.

Focused on prevention and early diagnosis, the Institute launches its campaign with the slogan “Be aware of the signs”.

Throughout the month, shades of green and #stayatentoaosinais will take care of the Institution and our social networks.

According to the National Cancer Institute (Inca), head and neck cancer appears as the fourth highest incidence. In Brazil, more than 40 thousand people receive the diagnosis each year. In 2020, Instituto Mário Penna performed more than 6,000 consultations and 762 surgeries for these patients.

Be aware that symptoms are very common and can be confused. They are: hoarseness, pain or difficulty in swallowing for more than 15 days, mouth sores that do not heal or nodules (lumps) in the neck.

To prevent, maintain good hygiene and oral health, have a balanced diet rich in nutrients. Avoid prolonged sun exposure, excess alcohol and do not smoke.

Our team is made up of renowned surgeons in head and neck cancer, including doctors Alvimar Afonso, Lysio França, Lorenzo Testolin, Rafael Malheiros and Sérgio Negri.

“The surgeon is the most suitable professional for diagnosing and staging tumors that affect the region, in addition to performing surgical treatment modalities. The physician has the essential role of helping the patient to decide the best form of treatment for each individual case”; explains Lorenzo Testolin, surgeon and head and neck surgery residency coordinator at Instituto Mário Penna.

In addition, it is up to the physician to indicate the intervention of the Institute’s multidisciplinary team – formed by psychologists, physiotherapists, stomatologists, speech therapists and nutritionists – who will monitor each patient, according to their need.

Our service

Head and neck cancer patients at Instituto Mário Penna also have the support of our multidisciplinary team, made up of psychologists, physiotherapists, stomatologists, speech therapists and nutritionists. Learn a little more about the importance of each one.

Psychology: The diagnosis of cancer has a great physical and emotional impact. We are ready to help the patient with doubts and support decisions. The patient does not need to go through this difficult time alone.

Physiotherapy: Cancer of the larynx (vocal cords) brings radical changes in the way you breathe and swallow. Thus, associated respiratory diseases are common. We have an innovative project that provides outpatient physical therapy consultations for specific monitoring of patients with laryngeal cancer. This work is extremely important in the patient’s recovery and, thus, reducing hospitalizations. Care encompasses respiratory rehabilitation through specific exercises and guidance to improve the quality of life of the patient and family members involved in the treatment.

Stomatology: It is the specialty of dentistry that prevents, diagnoses and treats diseases that manifest in the mouth. It is of paramount importance in the care of patients, especially those undergoing chemotherapy and/or radiotherapy. These treatments can evolve with mucositis, compromising oral function (chewing, swallowing and pain when swallowing). Through stomatological treatment with medication and laser therapy, the effects of cancer treatment are mitigated, making it faster and more comfortable for the patient.

Speech-language pathology: The speech-language pathologist is essential in the assessment and treatment of patients affected by head and neck cancer. Such diseases can compromise voice, speech and nutrition, requiring adjustments and rehabilitation, regardless of the treatment modality. With speech therapy rehabilitation, patients can regain their communication, pleasure and security when eating.

Nutrition: The evaluation of a nutritionist is extremely important during the illness. Early nutritional support can reduce weight loss before, during and after completion of treatment, providing better results and better quality of life. About 80% of cancer patients are malnourished at diagnosis. This is due to an imbalance between intake and nutrient requirements, and is associated with increased morbidity and mortality – worse outcome.

The discovery of laryngeal cancer could be the end of the voice for many people. For fourteen patients who lost their voice due to this type of cancer, it was the opportunity to learn to sing. They are part of the Mário Penna Institute’s Laryngectomized Choir and were reborn from singing.

The patients in the group underwent surgery for the total removal of the larynx – a procedure called “Laryngectomy” – completely losing their voice. However, with rehabilitation through esophageal voice acquisition or tracheoesophageal prosthesis implantation, they were able to resume communication.

The Mário Penna Institute, focused on excellence in care that includes diagnosis, treatment, rehabilitation and reintegration into society, is the only one in the capital that offers this service through the SUS.

These patients/singers have enchanted wherever they go and draw attention for the joy, pride and optimism with which they face life.

Electronic Larynx

The Mário Penna Institute reaches an important milestone in the history of SUS, being the first to receive Electronic Laryngs for patients undergoing surgery and who need rehabilitation for the use of their voice. The Mouth and Throat Cancer Association (ACBG) Brazil has been at the forefront of the fight to insert the device in the SUS table for years and approval was achieved in January 2021.

The electronic larynx, in addition to being a highly successful method in the rehabilitation process, provides the patient with conditions to resume their oral communication through the use of a device that produces a mechanical vibration and, when placed in contact with the skin in the region of neck, cheek or intraoral, produces a vocalization that is robotic and understandable, but in proportion to the tracheoesophageal prosthesis.

“It is a very important moment for Instituto Mário Penna. We have been fighting for years for this achievement by the SUS so that laryngectomized patients can speak again and express their feelings. Everyone will be covered, from the oldest to the newly operated. They will be trained in outpatient speech therapy, according to the rehabilitation program proposed and prepared by our team, which ranges from knowledge of the instrument’s potential to the acquisition of practice for its fluent use”; emphasizes Dr. Lorenzo Testolin.

August is the month to fight Lymphoma

August is Combat Lymphoma Month and many people know little or nothing about it. Therefore, the Mário Penna Institute supports the campaign and contributes to clarifying the disease in its media. Lymphomas are cancers that originate in the lymphatic system, a network of vessels and ganglia in our body responsible for storing the body’s defense cells.

Pedro Las Casas, a hematologist at the Instituto Mário Penna, explains that the disease arises when lymphocytes mutate and start to multiply in an uncontrolled manner, mainly inside the ganglia, causing them to increase in size. He points out that it is even possible that this proliferation occurs in other organs unrelated to the lymphatic system, such as the skin, lungs and nervous system.

“Lymphomas are divided into two types: Hodgkin’s and non-Hodgkin’s. Within these two groups it is possible to further divide them into several subtypes, each with completely different clinical characteristics and treatments. But only through the biopsy of the lesions is it possible to differentiate them”; account Dr. Pedro.

In general, the main symptoms of lymphomas are palpable and hardened nodes in the neck and armpits, which are usually painless and progressively growing. There are, however, other diseases that can lead to enlarged nodes, such as infections. Therefore, it is essential to seek a doctor for differential diagnosis if the nodes are progressively enlarged, are persistent or if they are accompanied by other symptoms such as persistent tiredness, daily fever, heavy sweating at night and weight loss.

The specialist also warns that for the diagnosis specific tests are needed, such as a biopsy of the ganglion or an organ suspected of being affected, blood tests and PET, a type of tomography that helps to identify the location of the lesions.

Dr. Pedro explains that the treatment of lymphomas varies according to the patient, the type of lymphoma and the degree of the disease. She points out that treatments are carried out through chemotherapy, immunotherapy and radiotherapy. “Unfortunately there is no form of prevention. When discovered at an early stage, the chances of cure or control are high. Therefore, it is important to always seek medical attention when symptoms are noticed”.

NEP conducts research related to head and neck cancer

World Head and Neck Cancer Awareness Day is celebrated on July 27th. One of the main problems for the treatment of this type of cancer is the late diagnosis, which occurs in 60% of cases, leaving sequelae in the patient.
We all want to avoid cancer, but this is not always easy. On the other hand, in head and neck cancer we can prevent the disease simply by avoiding exposure to factors that are related to the onset of the disease. There are well-known risk factors that can be easily avoided. The first factor: overexposure to sunlight. The second factor: cigarettes and alcohol and the situation is worse when the person has the habit of consuming large amounts of alcohol and smokes a lot. The third factor: HPV virus, the same one that is related to the appearance of cervical cancer in women.

In addition to prevention, early diagnosis and rapid initiation of treatment are essential for the cure of head and neck cancer, reducing the impact on patients’ quality of life. The Teaching and Research Center (NEP), through a partnership between the Translational Research Laboratory and the Federal University of Minas Gerais (UFMG), develops the research project “Association between clinical and immunopathological aspects of orofacial pain and quality of life in patients with squamous cell carcinoma of the oral mucosa”. The research is carried out by the master’s student Francine Barros de Oliveira, from the Postgraduate Program in Pathology at UFMG, under the guidance of Profa. Dr. Camila Megale Almeida-Leite (Dept of Morphology/Institute of Biological Sciences/UFMG) and co-supervision of Dr. Patricia Rocha Martins.

Dr. Paulo Guilherme de Oliveira Salles, Dr. Paulo Guilherme de Oliveira Salles, also participate in the research as members of the Mário Penna Institute (IMP) team. Juliana Maria Braga Sclauser and Dr. Letícia Braga, in addition to the medical team at the head and neck clinic. The project aims to evaluate in oral cancer patients seeking treatment at IMP, potential relationships between cancer characteristics, orofacial pain presented by the patient and quality of life.

According to Dr. Camila Megale, this research is the starting point for more detailed studies on the type of orofacial pain that patients with head and neck cancer have and how the type of cancer can interfere with pain as a symptom. “In the future, the results obtained will allow for an early treatment of the patient’s pain in a more specific way, based on information obtained from the biopsy and the initial consultations, which will make orofacial pain relief more effective.”

50 years of IMP is celebrated with well lived and new flag wing

Instituto Mário Penna closed the 50th anniversary celebrations with a golden key. The 28th of June began with the raising of the flags of Brazil, Minas Gerais and Instituto Mário Penna, at the entrance of the PA Agreement. To represent all employees, the flags went up to the sound of the National Anthem by Marco Antônio Viana Leite, CEO of IMP; Juarez Rodrigues, Administrative Assistant; and Celma Fátima Vieira, Coordinator of SAME.

Next, Marco Antônio went to all sectors distributing “well-lived” cookies to employees. Lunch for patients and companions was also special. The CEO made a point of passing by the beds delivering the Italian straw made with all the affection by the Nutrition and Dietetics Service (SND) team as a souvenir of the institution’s 50th anniversary.

And do not stop there. Congratulations went straight to the Mineirão Stadium at a beautiful sunset, with a live show by singer Flávio Venturini. Everything was broadcast live on the institution’s Instagram and Youtube and, whoever wants to review it, just access these social networks.
